Important Notice: India Enhances Entry Quarantine Measures

The Ministry of Health and Family Welfare of India has announced the strengthening of border controls and quarantine measures for travelers entering or transiting through India.

1. For All Passengers Bound for India

All international arrivals to India, regardless of nationality, are required to submit a "Self Declaration Format" upon arrival.

 

This form will be distributed at the boarding gate before departure or onboard the aircraft for flights bound for India.

Please ensure you receive a copy and complete it before landing.

Please note that every passenger must fill out their own form before arrival.

1. PART-Ⅰ: PERSONAL INFORMATION

Section required for all passengers to complete.

Required information: Name, Flight No., Seat No., Passport No., Nationality, Age, Arrival Date/Time, Origin, Final Destination, Places visited in India

2. PART-Ⅱ: CONTACT ADDRESS IN INDIA (Address of Stay During the Next 21 Days)

Required information: Address in India, Residence in departure country (only for foreign nationals), Contact in India, Alternative contact, Email address

3. PART-Ⅲ: TRAVEL AND HEALTH DECLARATION

Past travel history and current health status. Must be completed by everyone.

 

A. Travel History in the last 21 days

Indicate if you have visited or transited through affected countries.

Affected countries: DRC, Uganda, South Sudan (Based on WHO updates)

 

B. Confirmation of Contact History

Any contact with Ebola patients, lived with, nursed, visited hospital, or attended a funeral.

 

C. Current Symptoms

Select any that apply: No symptoms, fever, muscle pain, headache, vomiting, diarrhea, sore throat, or rash.

4. Declaration (Signature & Arrival Date)

Declare that information is true and sign. 

Includes agreement to comply with quarantine measures and official instructions.

 

2. Travelers Subject to Enhanced Quarantine Measures

These measures apply to all passengers arriving from or transiting through the following countries where Ebola cases have been reported.

  • Democratic Republic of the Congo (DRC)
  • Uganda
  • South Sudan

(Subject to change based on the pandemic situation.)

3. Quarantine Requirements Upon Arrival

Applicable travelers subject to the above enhanced quarantine measures are required to follow these procedures:

1. Duty to Declare Health Status

If you have symptoms (fever, headache, muscle pain, etc.) or had potential contact with an infected person's bodily fluids, you must report to the Airport Health Officer/Health desk immediately before immigration.

2. 21-Day Health Monitoring

Even if symptom-free, you must monitor your health for 21 days post-arrival. 

If symptoms appear, seek medical care immediately and inform healthcare authorities about  your travel history.
